| The Student Health Clinic is
available for University of Idaho
students and their dependants. The
clinic is located in the heart of the UI
campus at 831 Ash Street. Please
call 208.885.6693 to schedule your
appointment.
If you are a new patient to the
Student Health Clinic, please download a
Patient Information Form for you to
print, complete and bring with you to
your first appointment.
If the student is under age 18, a
parent or guardian will need to
authorize treatment. Please
download an
Advance Consent to Treat Minors Form
to print, complete and provide to the
Student Health Clinic.
We would be happy to forward copies
of your medical records to your
home-town physician. Please
download and print a
Request for Release of Medical Records
Form and bring it to the Student
Health Clinic. We will then
forward copies to your physician.
The clinic
is normally open for scheduled and walk-in
appointments:
Monday through Friday 8:00am to
5:00pm
Closed Saturday and Sunday
During Breaks and during the Summer
Session, the clinic is open for
scheduled and walk-in appointments:
Monday through Friday 8:30am to
3:00pm
