clinic. Please check our
medical staff directory page to find
a suitable match and then simply call
208.882.2011 for an appointment.
Our reception staff will ask you for
your name, date-of-birth, address,
current insurance and your return
telephone number prior to booking your
appointment. Prior to your first
appointment, please stop by our office
or download a
new patient registration form to
complete and bring with you to your
first appointment.
If you have medical records from your
previous provider, please bring copies
of that record to your first
appointment. Otherwise, stop by
our office or download a
release of records form which we
will use to request records from your
previous provider(s).
For your first appointment, please
bring either a list of your current
medicines with doses and how/when you
take them or put all your medications in
a bag and bring them in.
